Agencies that

Assisted Living Facility
Appropriate for
individuals who still have some ability to be independent, but may need assistance with some activities of daily living, medication
reminders, and meals.

Home Health Care
Licensed personnel i.e.
registered nurse, licensed vocational nurse, and physical, occupational, and speach therapists visit the patient in the home
to assist with rehabilitation. The services of a nurse aide and social worker may also be available depending on the health
of the patient.

Independent or Retirement Living Facility
For
persons who are independent, but would like their meals, cleaning, and a variety of activities available on site.

Hospice
Hospice is a benefit available to
the most frail patients who, if their disease process follows its normal course, have approximately six months to live. However,
sometimes patients remain on hospice for a longer period of time.

Skilled Nursing Facility
Appropriate when
the patient needs the services of 'skilled' personnel i.e. registered nurse, licensed vocational nurse, physical therapist,
occupational therapist, or speech therapist.
